ABSTRACT Purpose: to critically analyze whether the legislation regarding the Personal Frequency-Modulated (FM) System device encompasses most of the students with hearing impairment to facilitate the development of skills required for communication, literacy, and learning. Methods: a legislative analysis of the norms regarding the use of the Personal Frequency-Modulated (FM) System device. Relevant legislations were searched on public databases such as the Planalto and the Ministry of Health portals. Brazilian laws, ordinances, and relevant guidelines were consulted as well. Literature Review: Ordinance n. 1,274 of June 25, 2013, the CONITEC Report on FM in 2020 and GM/MS Ordinance No. 2,465, of September 27, 2021, which regulate the Personal Modulated Frequency System device, were identified. Conclusion: the initial ordinance of 2013, which regulated the Personal Frequency-Modulated (FM) System, was revised and updated, because it did not include children under six years of age, who are in the peak phase of oral language learning.

RESUMO Objetivo: analisar criticamente se a legislação garantidora do dispositivo Sistema de Frequência Modulada Pessoal abrange a população adequada de escolares com deficiência auditiva no critério idade, de forma a favorecer o desenvolvimento das habilidades necessárias para a comunicação, a alfabetização e a aprendizagem dessas crianças. Métodos: trata-se de uma análise legislativa das normas referentes ao uso do dispositivo Sistema de Frequência Modulada Pessoal. Para a busca da legislação pertinente, foram utilizados bancos de dados públicos, tais como: o portal do Planalto e o portal do Ministério da Saúde. Foram consultadas as leis, portarias e diretrizes Brasileiras no que se refere ao uso do Dispositivo Sistema de Frequência Modulada Pessoal. Revisão da Literatura: foram identificadas na Revisão de Literatura a Portaria n. 1.274 de 25 de junho de 2013, o Relatório do CONITEC sobre o FM no ano de 2020 e a Portaria GM/MS Nº 2.465, de 27 de setembro de 2021 que regulamentam o dispositivo Sistema de Frequência Modulada Pessoal. Conclusão: é evidente que a portaria inicial de 2013, a qual regulamentava o fornecimento do Sistema FM, foi revista e atualizada, uma vez que não contemplava crianças menores de 6 anos, em fase de máxima aprendizagem da linguagem oral, pré-alfabetização e letramento, mesmo que estivessem inseridas em instituições escolares.

ABSTRACT Purpose Longitudinally verify the influence of auditory tonal thresholds obtained with transcutaneous and percutaneous bone-anchored hearing aids on speech perception in individuals with external and/or middle ear malformation and chronic otitis media. Methods Observational, retrospective, longitudinal follow-up study of 30 unilateral users of the transcutaneous and percutaneous Baha® system for the collection of secondary data on pure tone thresholds obtained through free field audiometry and sentence recognition threshold in silence and noise in conditions: without the prosthesis; at the time of activation; in the first month of use (post 1); and in the third month (post 2). Results There was a significant difference between pure tone thresholds obtained at frequencies of 3 and 4kHz with better results for the percutaneous technique at all evaluation moments. For both systems, better performance was observed in sentence recognition in silence and in noise, with a significant difference in activation (p<0.001), but it remained stable during the other evaluation moments. The percutaneous system showed better benefit in recognizing sentences in noise only on activation (p=0.036), when compared to the transcutaneous system. Conclusion The percutaneous system provided better audibility for high frequencies; however, such audibility did not influence sentence recognition in the silent situation for both systems. For the noise situation, better responses were observed in the percutaneous system, however, the difference was not maintained over time.

RESUMO Objetivo Verificar longitudinalmente a influência dos limiares tonais auditivos obtidos com as próteses auditivas ancoradas no osso transcutâneas e percutâneas na percepção da fala em indivíduos com malformação de orelha externa e/ou média e Otite Média Crônica. Método Estudo observacional, retrospectivo, de seguimento longitudinal de 30 indivíduos usuários unilaterais de sistema Baha® transcutâneo e percutâneo, para coleta de dados secundários dos limiares tonais obtidos por meio da audiometria em campo livre e do limiar de reconhecimento de sentenças no silêncio e no ruído nas condições: sem a prótese; no momento de ativação; no primeiro mês de uso (pós 1); e no terceiro mês (pós 2). Resultados Houve diferença significante entre os limiares tonais obtidos nas frequências de 3 e 4kHz, com melhores resultados para o percutâneo em todos os momentos de avaliação. Para os dois sistemas, observou-se melhor desempenho no reconhecimento de sentenças no silêncio e ruído, com diferença significante na ativação (p<0,001), porém manteve-se estável nos demais momentos de avaliação. O sistema percutâneo mostrou melhor benefício no reconhecimento de sentenças no ruído apenas na ativação (p=0,036), quando comparado ao transcutâneo. Conclusão O sistema percutâneo possibilitou melhor audibilidade para as frequências altas; contudo, tal audibilidade não influenciou no reconhecimento de sentenças na situação de silêncio para ambos os sistemas. Para a situação de ruído, melhores respostas foram observadas no sistema percutâneo; porém, a diferença não se manteve no decorrer do tempo.

ABSTRACT Purpose To evaluate the usability and satisfaction of users with the interface of the ‘Agente Escuta’ application, in addition to identifying problems and possibilities for improvement. Methods Descriptive exploratory translational study, characterized by a usability test with a quantitative and qualitative approach, subdivided into three stages: (I) prior evaluation of usability by 10 judges, including students, primary care professionals, professors and researchers in Information Technology and Speech Therapy; (II) evaluation of the application by the target audience, that is, community health agents from six municipalities in Rio Grande do Norte; (III) evaluation of the satisfaction of the agents who used the application in their work routine. The System Usability Scale and the Net Promoter Score were used, in addition to a qualitative evaluation of the opinions. Results Usability was rated as excellent by judges, regardless of category. In the evaluation by community health agents, usability was considered good and there was no effect of the city of origin. It was found that the perception of the judges and the target audience were different, with a lower score for the participants in the second stage. However, most would give positive publicity to the product. The heuristic with the highest score was ease of memorization and participants in the third stage were interested in continuing to use the tool in practice, even after the end of the study. Conclusion The Agente Escuta prototype showed good usability and satisfaction and aspects that could be improved in future solutions were identified.

RESUMO Objetivo Avaliar a usabilidade e satisfação dos usuários com a interface do aplicativo “Agente Escuta”, além de identificar problemas e possibilidades de melhorias. Método Estudo translacional exploratório descritivo, caracterizado por um teste de usabilidade com abordagem quanti-qualitativa, subdividido em três etapas: (I) avaliação prévia da usabilidade por 10 juízes, incluindo estudantes, profissionais da atenção primária, docentes e pesquisadores em Tecnologia da Informação e Fonoaudiologia; (II) avaliação do aplicativo pelo público-alvo, agentes comunitários de saúde de seis municípios potiguares; (III) Avaliação da satisfação dos agentes que utilizaram o aplicativo na rotina de trabalho. Utilizou-se o System Usability Scale e pelo Net Promoter Score, além de uma avaliação qualitativa das opiniões. Resultados A usabilidade foi avaliada como excelente pelos juízes, independentemente da categoria. Na avaliação pelos agentes comunitários de saúde, a usabilidade foi considerada boa e não houve efeito do município de origem. Constatou-se que a percepção dos juízes e do público-alvo foram distintas, com menor escore para os participantes da segunda etapa, no entanto, a maioria dariam publicidade positiva ao produto. A heurística com maior pontuação foi a facilidade de memorização e os participantes da terceira etapa mostraram-se interessados em continuar utilizando a ferramenta na prática, mesmo após o término do estudo. Conclusão O protótipo do Agente Escuta apresentou boa usabilidade e satisfação e foram identificados aspectos que poderão ser aprimorados em soluções futuras.

ABSTRACT Purpose: to analyze the implementation and effectiveness of a hearing loss identification and intervention program in the first years of life in primary care. Methods: an experimental study carried out in two Basic Health Units. Twenty-three Community Health Workers participated, trained through an online self-instructional course from the Ministry of Health. Subsequently, they followed the hearing and language of children from zero to two years old, referring them, if necessary, to audiological evaluation. For the analysis of pre and post online course knowledge, the Wilcoxon test was applied, and to compare the performance between the participants of the two Basic Health Units, the Mann Whitney test was used, adopting p<0.05. Additionally, the qualitative perception of participating community workers, the referrals, and the result of the audiological evaluation were analyzed. Results: the online course was effective in improving the knowledge of the participants. A total of 102 children were monitored, 15 of them were referred for evaluation. From these, four underwent hearing screening and 11 underwent a complete hearing assessment, a case of conductive hearing loss being identified. Conclusion: the implementation of a program for the identification and intervention of hearing loss, in the first years of life, in primary care, permeated by community health workers, is feasible.

RESUMO Objetivo: analisar a implantação e a eficácia de um programa de identificação e intervenção da perda auditiva nos primeiros anos de vida na atenção primária. Métodos: estudo experimental realizado em duas Unidades Básicas de Saúde. Participaram 23 agentes comunitários de saúde, capacitados por meio de um curso autoinstrucional online do Ministério da Saúde. Posteriormente, acompanharam a audição e a linguagem das crianças de zero a dois anos, encaminhando-as, se necessário, para avaliação audiológica. Para análise do conhecimento pré e pós curso online foi aplicado o teste de Wilcoxon e para comparação do desempenho entre os participantes das duas Unidades Básicas de Saúde empregou-se o teste de Mann Whitney, com adoção de p<0,05. Adicionalmente, avaliou-se a percepção qualitativa dos agentes comunitários participantes, os encaminhamentos realizados e o resultado da avaliação audiológica. Resultados: o curso online foi eficaz na melhoria do conhecimento dos participantes. Totalizaram-se 102 crianças acompanhadas, 15 delas encaminhadas para avaliação. Destas, quatro realizaram triagem auditiva e 11 realizaram a avaliação auditiva completa, sendo identificado um caso de perda auditiva condutiva. Conclusão: a implantação de um programa de identificação e intervenção da perda auditiva nos primeiros anos de vida na atenção primária, permeada por agentes comunitários de saúde, mostrou-se viável.

RESUMO Objetivo Descrever os benefícios nos limiares auditivos e no desempenho de reconhecimento de sentenças no silêncio e no ruído em indivíduos com a adaptação unilateral do Sistema Ponto®. Métodos Estudo observacional, retrospectivo, de seguimento longitudinal. A casuística foi composta por fontes de dados secundários de dez indivíduos com perda auditiva condutiva ou mista, que foram submetidos à cirurgia com o Sistema Ponto®. Os resultados foram analisados nas seguintes condições: a) pré-cirúrgicas: sem AASI; com AASI por condução aérea ou óssea e com o processador Ponto Pro® acoplado a uma banda elástica; b) pós-cirúrgicas: na ativação e após seis meses de uso. Resultados Os limiares da audiometria tonal por conduções aérea e óssea mantiveram-se estáveis após a cirurgia, enquanto os limiares auditivos em campo livre e o reconhecimento de fala no silêncio e no ruído foram estatisticamente melhores na ativação e após seis meses de uso do Sistema Ponto®. Não houve diferença nos resultados com os indivíduos utilizando o Sistema Ponto® com a banda elástica e após a cirurgia. Conclusão O Sistema Ponto® propiciou benefício nas habilidades auditivas de detecção em todas as frequências testadas, assim como no reconhecimento de sentenças no silêncio e no ruído.

ABSTRACT Purpose To describe the benefits in hearing thresholds and sentence recognition performance in silence and noise, in users of the unilateral Ponto® system. Methods An observational, retrospective, longitudinal study. The sample consisted of secondary data sources from 10 individuals with conductive or mixed hearing loss who underwent surgery with the Ponto® System. The results were analyzed in the following pre-surgical conditions (without hearing aids; with hearing aids by air or bone conduction; with the Ponto Pro® processor with a soft band) and post-surgical (on activation and after six months of use). Results The thresholds of pure tone audiometry by air and bone conductions remained stable after surgery, while the auditory thresholds in free field and speech recognition in silence and in noise were statistically better when using the Ponto® system. There was no difference between the results obtained with the individuals using Ponto® with soft band and post-surgically. Conclusion The Ponto® system provided benefits in hearing detection skills in all tested frequencies, as well as, in recognition of the sentence in silence and noise.

Resumo Introdução O implante coclear é um tratamento eficaz para crianças com deficiência auditiva pré-lingual de grau severo e/ou profundo, uma vez que proporciona melhoria considerável na aquisição da linguagem oral por meio da via auditiva. O uso do implante coclear contribui para o desenvolvimento da percepção auditiva e favorece a aquisição dos processos linguísticos voltados à aquisição de habilidades comunicativas que poderá repercutir positivamente em outras áreas do desenvolvimento. Objetivo Verificar o desempenho de crianças usuárias de implante coclear quanto à linguagem oral expressiva e receptiva. Método Estudo prospectivo transversal onde os seguintes testes foram usados: teste de linguagem infantil nas áreas de fonologia, vocabulário, fluência e pragmática e teste de vocabulário de imagens Peabody. Participaram deste estudo 30 crianças de ambos os sexos de 36 a 72 meses, com deficiência auditiva sensorioneural de grau severo e/ou profundo bilateral, sem outros comprometimentos e usuárias de implante coclear unilateral com inserção total dos eletrodos havia, no mínimo, 12 meses. Resultados O maior tempo de uso do implante coclear, a menor idade na cirurgia e o melhor desempenho na percepção auditiva da fala influenciaram o desempenho na linguagem oral expressiva e receptiva. Embora comparativamente ao processo de aquisição normativo da linguagem os resultados encontrados tenham revelado que essas crianças demonstraram habilidades linguísticas em padrões abaixo de sua idade cronológica, os resultados apontaram que essas crianças estão em evolução da linguagem oral expressiva e receptiva, esse é o resultado que deve ser valorizado neste estudo. Conclusão O maior tempo de uso do implante coclear, a menor idade na cirurgia e o melhor desempenho na percepção auditiva da fala influenciaram o desempenho na linguagem oral expressiva e receptiva, porém não em todas as categorias semânticas estudadas.

Abstract Introduction The cochlear implant is an effective device for children with severe and/or profound prelingual hearing loss, since it provides considerable improvement in oral language acquisition through the auditory pathway. The use of a cochlear implant contributes to the development of auditory perception, favoring the acquisition of the linguistic processes related to communication skills, which might have a positive effect on other areas of development. Objective The aim of this study was to verify the performance of children using cochlear implants for expressive and receptive oral language. Methods This was a prospective cross-sectional study that used the following tests: Child language test in the phonology, vocabulary, fluency and pragmatics areas, and the Peabody picture vocabulary test. Thirty children participated in this study, of both genders, aged between 36 and 72 months, with severe and/or profound bilateral sensorineural hearing loss, without other impairments and users of unilateral cochlear implant with full electrode insertion for a minimum of 12 months. Results The longer duration of the cochlear implant use, the younger age at surgery and the better performance in the auditory perception of speech influenced the performance in expressive and receptive oral language. Even though when compared to the normative language acquisition process, the results showed that these children had patterns of linguistic skills that are below their chronological age; the results indicate that these children are developing expressive and receptive oral language skills, and this is the outcome that should be taken into account in this study. Conclusion The longer duration of the cochlear implant use, the younger age at surgery and the better performance in the auditory perception of speech influenced the performance in expressive and receptive oral language skills, but not in all the studied semantic categories.

RESUMO Objetivo Descrever a perda auditiva sensorioneural unilateral e bilateral assimétrica em crianças quanto às características etiológicas, audiológicas e demográficas. Método Estudo retrospectivo transversal, desenvolvido na Seção de Implante Coclear do Hospital de Reabilitação de Anomalias Craniofaciais, por meio da análise de prontuários. Resultados Foram analisados os dados de 1152 pacientes, sendo 424 (37%) adolescentes, adultos ou idosos e 728 (63%) crianças, dentre as quais, 691 (95%) apresentavam perda auditiva bilateral simétrica e 37 (5%) perda auditiva sensorioneural unilateral (n=10) ou bilateral assimétrica (n=27). A idade média ao diagnóstico na perda auditiva sensorioneural unilateral foi de 33,58±21,69 meses e na bilateral assimétrica de 33,12±21,69 meses, com prevalência de 1,4% e 3,7%, respectivamente. O indicador de risco para a deficiência auditiva de maior ocorrência para ambos os grupos foi o de antecedente familiar. A maioria dos familiares das crianças com perda auditiva sensorioneural unilateral apresentaram a classificação socioeconômica baixa superior (50%), enquanto que as crianças com perda auditiva sensorioneural bilateral assimétrica se subdividiram igualmente em baixa superior (37%) e média inferior (37%). Conclusão Houve uma maior prevalência da perda auditiva sensorioneural bilateral assimétrica em relação à unilateral, bem como do indicador de risco de hereditariedade, com predomínio do grau profundo na pior orelha e preponderância do sexo feminino, em ambos os grupos. Apesar de a triagem auditiva neonatal propiciar a identificação precoce da perda auditiva sensorioneural unilateral, a idade no diagnóstico audiológico ainda se encontra acima do recomendado. Adicionalmente, a maioria dos familiares das crianças apresentou nível de rendimento baixo.

ABSTRACT Purpose To describe unilateral and bilateral asymmetric sensorineural hearing loss in children and its etiological, audiological and demographic characteristics. Methods Retrospective cross-sectional study developed in the Seção de Implante Coclear of Hospital de Reabilitação de Anomalias Craniofaciais, through the analysis of medical records. Results Data from 1152 patients were analyzed: 424 (37%) adolescents, adults or elderly, and 728 (63%) children, of whom 691 (95%) had bilateral symmetrical hearing loss, and 37 (5%) had unilateral hearing loss (n=10) or bilateral asymmetric (n=27) sensorineural hearing loss. The mean age at diagnosis of unilateral sensorineural hearing loss was 33.58±21.69 months, and for asymmetric bilateral it was 33.12±21.69 months, with a prevalence of 1.4% and 3.7%, respectively. The highest risk indicator for hearing loss for both groups was the family history of permanent deafness, which began in childhood. The majority of the relatives of children with unilateral sensorineural hearing loss presented the highest low socioeconomic classification (50%), while children with bilateral asymmetric sensorineural hearing loss were also be subdivided into upper (37%) and lower (37%). Conclusion We observed a greater occurrence of asymmetric bilateral sensorineural hearing loss compared to unilateral hearing loss, as well as the hereditary risk indicator, with a predominance of the deep ear and female preponderance in both groups. Although neonatal hearing screening provides early identification of unilateral sensorineural hearing loss, the age at the audiological diagnosis is still above the recommended level. In addition, the majority of the children’s family members presented a low level of income.

RESUMO Objetivos analisar criticamente a necessidade do uso do mascaramento contralateral na pesquisa do potencial evocado auditivo de tronco encefálico (PEATE) por condução aérea, na perda auditiva sensorioneural unilateral ou bilateral assimétrica, por meio de uma revisão sistemática da literatura. Estratégia de pesquisa foram consultadas as bases de dados eletrônicas PubMed/MEDLINE, LILACS, ADOLEC, IBECS, SciELO, Web of Science, Scopus e Embase, bem como os anais do Encontro Internacional de Audiologia e do Congresso Brasileiro de Fonoaudiologia, além das bases de instituições com pós-graduação em Fonoaudiologia, utilizando os descritores Evoked Potentials, Auditory, Brain Stem, Perceptual Masking e Hearing Loss, Sensorineural. Critérios de seleção foram selecionados os estudos com níveis de evidência de 1 a 5, publicados em português, inglês ou espanhol, até janeiro de 2018. Os artigos deveriam abordar a necessidade do uso do mascaramento contralateral na pesquisa do PEATE por condução aérea, em indivíduos com perda auditiva sensorioneural unilateral ou bilateral assimétrica. Resultados foram encontrados 334 estudos, sendo que oito artigos contemplaram os critérios preestabelecidos. Os estudos incluídos foram controversos quanto ao uso do mascaramento contralateral. Conclusão não existe um consenso quanto à necessidade do uso do mascaramento contralateral no PEATE por condução aérea, em indivíduos com perda auditiva sensorioneural unilateral. No entanto, a maioria dos estudos direcionou para a sua utilização na pesquisa do PEATE com estímulo clique em indivíduos com perda auditiva sensorioneural unilateral de graus severo e profundo.

ABSTRACT Purpose Critically analyze the need to use contralateral masking in the measurement of Auditory Brainstem Response (ABR) by air conduction on unilateral or bilateral asymmetric sensorineural hearing loss by means of a systematic literature review. Research strategies A search was conducted at the PubMed/MEDLINE, LILACS, ADOLEC, IBECS, SciELO, Web of Science, Scopus and Embase electronic databases, proceedings of the International Meeting of Audiology and the Brazilian Congress of Speech-language Pathology and Audiology, and bases of institutions with Graduate Studies Programs in Speech-language Pathology and Audiology, using the following descriptors: Evoked Potentials, Auditory, Brainstem, Perceptual Masking, Hearing Loss, and Sensorineural. Selection criteria Studies with levels of evidence from 1 to 5 published in Portuguese, English or Spanish until January 2018 were selected. The articles should address the need to use contralateral masking in the measurement of air-conduction ABR in individuals with unilateral or bilateral asymmetric sensorineural hearing loss. Results A total of 334 studies were found, and eight of them met the pre-established criteria. The studies included in the review were controversial as to the need for contralateral masking. Conclusion There is no consensus on the need to use contralateral masking in the measurement of ABR by air conduction in individuals with unilateral sensorineural hearing loss; however, most studies report the use of contralateral masking in the measurement of ABR with click stimulus in individuals with severe and profound unilateral sensorineural hearing loss.

RESUMO Objetivo Verificar a influência da idade de implantação no desenvolvimento das habilidades de reconhecimento auditivo em conjunto fechado e compreensão auditiva em crianças usuárias de implante coclear unilateral, comparando grupos distintos e determinar os marcadores clínicos de desenvolvimento destas habilidades. Método Participaram do estudo 180 crianças operadas e ativadas até os 36 meses de idade e que utilizaram o IC durante, no mínimo, 60 meses. Foram analisadas as habilidades de reconhecimento auditivo em conjunto fechado e compreensão auditiva por meio das Provas 5 e 6 do GASP. Para investigar a influência da idade na implantação, as crianças foram divididas em três grupos: implantadas antes dos 18 meses (G1), entre 19 e 24 meses (G2) e entre 25 e 36 meses de idade (G3). Resultados Não houve diferença estatisticamente significante quando comparados os três grupos. As crianças desenvolveram progressivamente as habilidades auditivas, apresentando a habilidade de reconhecimento auditivo em conjunto fechado por volta dos 41±4 meses de uso do IC e a de compreensão auditiva por volta dos 53±4 meses. Conclusão Não houve correlação entre o desempenho auditivo e a idade de implantação para as crianças implantadas antes dos 36 meses de idade. Para as habilidades de reconhecimento e compreensão auditivas, o marcador clínico foi aos 41±4 e aos 53±4 meses de idade auditiva, respectivamente. Para tanto, espera-se que, por volta dos 60 meses de uso do IC, as crianças implantadas durante o período sensível possam compreender a fala sem o auxílio da leitura orofacial, alcançando as habilidades auditivas mais complexas.

ABSTRACT Purpose To verify the influence of the age of implantation in the development of closed-set auditory recognition and auditory comprehension abilities in children using unilateral cochlear implants (CI), comparing distinct groups and determining clinical markers. Methods Participants were 180 children operated and activated until 36 months of age and who used a CI for at least 60 months. Abilities of auditory recognition in closed-set and auditory comprehension were analyzed through the GASP Tests 5 and 6. The influence of age of implantation was investigated with three groups of children: implanted before 18 months (G1), between 19 and 24 months (G2) and between 25 and 36 months of age (G3). Results There was no statistically significant difference when comparing the three groups. Children progressively developed auditory abilities, presenting auditory recognition ability together at approximately 41±4 months of CI use and auditory comprehension at 53±4 months. Conclusion There was no correlation between hearing performance and age of implantation for children implanted before 36 months of age. For the abilities of auditory recognition and comprehension, the clinical marker was 41±4 and 53±4 months of auditory age, respectively. Therefore, it is expected that, around 60 months of CI use, children implanted during the sensitive period can understand speech without the aid of orofacial reading, reaching the most complex hearing abilities.

RESUMO A deficiência auditiva pode estar relacionada com diversos fatores, entre eles, a perda auditiva decorrente de determinadas síndromes genéticas. A síndrome Kearns-Sayre é caracterizada por mutações no DNA (deoxyribonucleic acid) mitocondrial, responsável pela produção de energia (trifosfato de adenosina - ATP), que é de extrema importância para o desenvolvimento de estruturas que dela necessitam, como a cóclea. O caso foi acompanhado no hospital, desde o ano 2000, devido à característica progressiva da perda auditiva constatada nos exames audiológicos e nos achados em casos relacionados à síndrome. A intervenção com os aparelhos de amplificação sonora individual (AASI) se mostrou pouco benéfica para a boa comunicação oral de uma das pacientes, que passou a ter o diagnóstico de perda auditiva profunda bilateral. Assim, após discussões em reuniões clinicas, a equipe optou pela indicação do implante coclear para a paciente, segundo os critérios atuais para indicação desta cirurgia, e com o qual obteve bons resultados. Sua irmã gêmea, que apresentou bons resultados com AASI, continuará em acompanhamento audiológico, para se verificar a evolução do caso e discutir uma nova conduta, caso necessário. Pacientes com suspeita, ou diagnóstico de síndrome Kearns-Sayre devem buscar o diagnóstico audiológico, pois se trata de uma possível perda auditiva progressiva, sendo necessária a reabilitação com o uso de dispositivos auditivos. Manter a comunicação oral é de extrema importância, pois, nestes casos, outras funções serão prejudicadas, como o tônus muscular e a visão.

ABSTRACT Hearing impairment may be related to several factors, including hearing loss due to certain genetic syndromes. Kearns-Sayre syndrome is characterized by mutations in mitochondrial DNA (deoxyribonucleic acid), responsible for energy production (adenosine triphosphate - ATP), which is extremely important for the development of structures that need it, such as the cochlea. The case was followed in the hospital, since 2000, due to the progressive characteristic of the hearing loss observed in the audiological examinations and the findings in cases related to the syndrome. The intervention with individual sound amplification devices (AASI) proved to be of little benefit to the good oral communication of one of the patients, who was diagnosed as having bilateral profound hearing loss. Thus, after discussions at clinical meetings, the team opted for the indication of the cochlear implant for the patient, according to the current criteria for indication of this surgery, and with which it obtained good results. Her twin sister, who presented good results with AASI, will continue to undergo audiological follow-up, to verify the evolution of the case and discuss a new conduct, if necessary. Patients with suspected or diagnosed Kearns-Sayre syndrome should seek the audiological diagnosis, since it is a possible progressive hearing loss, requiring rehabilitation with the use of hearing devices. Maintaining oral communication is extremely important because in these cases other functions will be impaired, such as muscle tone and vision.

RESUMO Introdução Pesquisas voltadas para as populações com baixas exposições a solventes, ou exposições dentro dos níveis de tolerância permitidos em âmbito ocupacional ainda são restritas. Objetivo Caracterizar o perfil auditivo de frentistas de postos de combustíveis. Métodos Estudo transversal, constituído por dois grupos, pareados por gênero e idade: Grupo Controle - 23 indivíduos sem exposição a ruído ou agentes químicos; Grupo Experimental - 21 frentistas de postos de combustíveis. Foi realizada avaliação audiológica, composta por audiometria tonal liminar, logoaudiometria e medidas de imitância acústica. A análise estatística utilizou cálculo de média, desvio padrão, valor mínimo e máximo, teste Qui-quadrado e coeficiente de correlação de Pearson. O nível de significância adotado foi de 5%. Resultados Foram verificadas alterações auditivas nos frentistas, caracterizadas por comprometimento no sistema auditivo periférico, sugerindo ação tóxica da exposição a combustíveis. Houve correlação entre idade e tempo de exposição a solvente. Na comparação entre os grupos, o reflexo acústico demonstrou maior número de alterações no grupo experimental, com diferença para os reflexos acústicos ipsilaterais da orelha direita e contralaterais da orelha esquerda. Conclusão Não houve diferença entre os grupos para os limiares auditivos, porém, a diferença verificada nos reflexos acústicos ipsilaterais e contralaterais no grupo experimental sugere comprometimento retrococlear. Diante das evidências observadas neste estudo, considera-se relevante incluir a pesquisa do reflexo acústico na avaliação auditiva dos frentistas, bem como a integração desta categoria profissional aos programas de prevenção de perda auditiva.

ABSTRACT Introduction Researches into populations with low solvent exposures, or exposures within tolerance levels allowed in the occupational field are still restricted. Purpose To characterize the hearing profile of gas station attendants. Methods Cross-sectional study, constituted of two groups, matched by gender and age: Control Group - 23 subjects without exposure to noise or chemicals; Experimental Group - 21 gas station attendants. An audiological evaluation was performed, composed by pure tone audiometry, speech audiometry and acoustic impedance tests. The statistical analysis used average calculation, standard deviation, minimum and maximum value; Chi-square Test and Pearson’s correlation coefficient. The significance level was 5%. Results Hearing loss was verified in gas station attendants characterized by impairment of the peripheral auditory system, suggesting toxic effects of exposure to fuels. There was a correlation between age and solvent exposure time. Comparing the groups, the acoustic reflex showed more alterations in the experimental group, with a difference for the ipsilateral acoustic reflexes of the right ear and contralateral ones of the left ear. Conclusion There was no difference between the groups for the hearing thresholds; however, the difference observed in the ipsilateral and contralateral acoustic reflexes in the experimental group suggests retrocochlear impairment. Due to the evidence observed in this study, it is considered relevant to include the acoustic reflex research in the auditory evaluation of the gas station attendants, as well as the integration of this professional category into hearing loss prevention programs.

RESUMO Objetivo Verificar os hábitos e atitudes de jovens em idade escolar com relação ao uso de estéreos pessoais e avaliar o nível de conhecimento sobre os efeitos nocivos da exposição à música amplificada na audição, antes e após uma palestra educativa. Métodos Participaram 58 estudantes do ensino fundamental e médio, entre 10 e 17 anos de idade. Foi realizada palestra educativa, mediante aula expositiva dialogada, utilizando recursos audiovisuais. Para avaliação foram aplicados dois questionários no formato online, antes e após 15 dias da palestra. Realizou-se, também, análise descritiva. Resultados A maioria dos escolares relatou fazer uso de estéreos pessoais com fone de ouvido, acima da intensidade média do equipamento e por período inferior a três horas diárias. A queixa mais frequente foi o zumbido. Após a palestra educativa, houve diferença no conhecimento sobre o conceito de perda auditiva e de que música em intensidade elevada, em momentos de lazer, pode causar perda auditiva. Também houve diferença na intensidade utilizada para ouvir música com fone de ouvido, que foi reduzida. Conclusão Grande parte dos escolares possuía conhecimento prévio a respeito dos prejuízos da música amplificada, para a audição e, ainda assim, alguns referiram hábitos inadequados. A palestra educativa revelou mudanças significativas em apenas alguns aspectos investigados, evidenciando a necessidade de estruturar ações contínuas na prevenção de perdas auditivas nessa população. A inclusão da escola e da família nas ações educativas e a continuidade dessas ações são fundamentais para mudanças efetivas de hábitos e atitudes das crianças e adolescentes.

ABSTRACT Purpose To verify the habits and attitudes of school-age youths regarding the use of personal music players, and assess their knowledge level about the harmful effects to the hearing of exposure to amplified music before and after an educational lecture. Methods A total of 58 elementary and middle school students aged 10 to 17 years participated in the study. We presented an educational lecture using an expository dialogue format with audiovisual resources. To evaluate the students, we applied two online questionnaires before and 15 days after the lecture. Descriptive analysis was also performed. Results Most students reported using personal music players with earphones above the mean intensity of the equipment and for fewer than 3 hours a day. The most frequent complaint was tinnitus. After the educational lecture, there were differences in knowledge about the concept of hearing loss and the fact that high-intensity music in moments of leisure may cause hearing loss. There was also a difference in the intensity used to listen to music with earphones, which decreased. Conclusion Most students had prior knowledge of the harmful effect of amplified music on hearing and even then, some indicated inadequate habits. The educational lecture showed significant changes in only some of the aspects investigated, reflecting a need to structure continuous actions towards hearing loss prevention in this population. The inclusion of the school and the family in educational actions and the continuity of these actions are fundamental to effective changes in habits and attitudes among children and adolescents.

RESUMO Objetivo: Caracterizar os valores de reflectância e absorvância de banda larga em lactentes com integridade de orelha média. Métodos: Foram analisados os valores de reflectância e absorvância de banda larga, para os estímulos chirp e tom puro, de 31 orelhas de 18 lactentes, na faixa etária de 10 dias a cinco meses de idade. Considerou-se como critérios de inclusão: ausência de fator de risco para deficiência auditiva, timpanometria com sonda de 1000 Hz, sugerindo normalidade de orelha média e presença de emissões otoacústicas evocadas por estímulo transiente, na triagem auditiva neonatal. Resultados: Os valores de reflectância mostraram-se maiores para as baixas frequências, reduzindo para as médias e aumentando, novamente, para as frequências altas, enquanto para os valores de absorvância, o comportamento foi o oposto. Não existiu diferença significativa entre os estímulos chirp e tom puro. Conclusão: Observou-se um comportamento típico das medidas estudadas, caracterizado por maior reflectância nas frequências graves e maior absorvância para as frequências médias.

ABSTRACT Purpose: To characterize the wideband reflectance and absorbance values in infants with middle ear integrity. Methods: The wideband reflectance and absorbance values for chirp and pure tone stimuli of 31 ears of infants aged 10 days to five months were analyzed. Inclusion criteria considered: 1000 Hz tympanometry. suggesting normal middle ear and the presence of transient evoked otoacoustic emission. Results: The reflectance values were shown to be greater for low frequencies, reducing to medium ones and increasing once again to high frequencies, while the absorbance values displayed an opposite behavior. No significant difference was verified between chirp and pure tone stimuli. Conclusion: A typical behavior of the measures, characterized by a higher reflectance at low frequencies and higher absorbance at medium frequencies was observed in the population studied.

RESUMO Objetivo: Verificar os aspectos de qualidade técnica e o conteúdo doswebsites sobre triagem auditiva neonatal na Língua Portuguesa. Métodos: Dezoito fonoaudiólogos, convidados de acordo com critérios de inclusão, selecionaram os descritores para a pesquisa dos websitespor meio da Técnica Delphi. Posteriormente, foram inseridos no Google Trends a fim de se acrescentar os termos possíveis de utilização pelos pais na busca de informações na internet sobre o assunto. Em seguida, foram inseridos no Google para pesquisa dos websites . Foram utilizados os seguintes instrumentos de avaliação: lista de tópicos sobre triagem auditiva neonatal, fórmula Flesch Reading Ease Score ,questionário Health-Related Web Site Evaluation Form Emory e o PageRank . Resultados: Os tópicos mais abordados nos 19 websites foram sobre os objetivos e benefícios da triagem auditiva neonatal, assim como o processo de diagnóstico audiológico. Os menos discutidos foram sobre o resultado falso-negativo, desenvolvimento da audição e da linguagem, resultado falso-positivo, acompanhamento audiológico, interpretação dos resultados - "Passa" / "Não passa", reteste e protocolo. Prevaleceu um nível de leitura dos textos considerado difícil, sendo os aspectos de qualidade técnica considerados de melhor qualidade os relacionados ao conteúdo, público, navegação e estrutura. Os resultados também demonstraram não existir uma cultura de inserir links noswebsites nacionais, o que os fizeram ser considerados de pouca relevância no Google. Conclusões: Os websites diferiram quanto aos aspectos abordados, assim como, há necessidade de revisar o nível de leitura dos conteúdos e os aspectos de qualidade técnica referentes à precisão e atualização das informações, autoria e links .

ABSTRACT Purpose: To verify the aspects of technical quality and the content of websites on neonatal hearing screening in Portuguese. Methods: Eighteen audiologists, invited to participate according to the inclusion criteria, selected descriptors of websites for research using the Delphi technique. Later, they were fed into Google Trends to get the possible terms to be used by parents in finding information on the Internet about the subject. They were then fed into Google to search the websites. The following assessment instruments were used: list of topics on newborn hearing screening, Flesch Reading Ease Score Formula, Health-Related Web Site Evaluation Emory Form, and PageRank. Results: The most discussed topics in the 19 websites were on the objectives and benefits of neonatal hearing screening, as well as the process of audiological diagnosis. The least discussed were about the false-negative result, development of hearing and language, false-positive results, audiologic, interpretation of results - "Pass"/"Do not pass", retest, and protocol. Difficult reading level was prevalent, with aspects of technical quality considered the best quality-related content, audience, navigation, and structure. The results also showed there is no culture of inserting links on Brazilian national websites, so they had little relevance on Google. Conclusions: The sites differed in the aspects addressed because there is a need to revise the reading level of the content and quality of the technical aspects regarding the accuracy and timeliness of information, authorship, and links.

OBJETIVO: Verificar a ocorrência e o limiar do reflexo acústico em neonatos utilizando a sonda de 226 e 1.000 Hz. MÉTODOS: Trinta e seis neonatos com resultado "PASS" na triagem auditiva neonatal e timpanogramas com um ou dois picos para os dois tons de sonda. O Grupo I foi composto por 20 neonatos a termo sem indicador de risco para deficiência auditiva e o Grupo II, por 16 neonatos com pelo menos um indicador de risco. A pesquisa dos limiares do reflexo acústico ipsilateral foi realizada em 500, 1.000, 2.000 e 4.000 Hz. RESULTADOS: Os grupos apresentaram limiar do reflexo acústico entre 50 e 100 dB para ambos os tons de sonda. Na comparação entre as sondas, houve diferença em todas as frequências avaliadas no Grupo I, com a média de limiares menor com a sonda de 1.000 Hz. No Grupo II, foi detectada diferença em 2.000 Hz. Com a sonda de 226 Hz, os limiares médios do reflexo acústico foram semelhantes nos dois grupos. Com a sonda de 1.000 Hz, houve diferença em todas as frequências avaliadas. A porcentagem de presença de resposta foi maior para ambos os grupos para a sonda de 1.000 Hz. O teste kappa revelou concordância extremamente pobre na comparação dos resultados entre as duas sondas. CONCLUSÃO: A ocorrência de reflexo acústico em neonatos foi maior e os limiares menores com a utilização da sonda de 1.000 Hz, tanto para neonatos saudáveis como para os neonatos de risco.

PURPOSE: To analyze the occurrence of acoustic reflex and its threshold on newborns using the 226 and 1,000 Hz probes. METHODS: Thirty-six newborns with "PASS" results in newborn hearing screening and tympanogram with one or two peaks for both probe tones were included. Group I comprised 20 full-term newborns without risk indicator for hearing loss, and Group II comprised 16 newborns with at least one risk indicator. The study about ipsilateral acoustic reflex thresholds was conducted in 500, 1,000, 2,000, and 4,000 Hz. RESULTS: The groups presented the acoustic reflex thresholds between 50 and 100 dB for both probe tones. In the comparison between the probes, there were differences in all frequencies evaluated in Group I, with the lowest threshold mean for the 1,000 Hz probe. In Group II, differences were detected at 2,000 Hz. The mean acoustic reflex thresholds were similar in both groups for the 226 Hz probe. There was a difference for the 1,000 Hz probe in all tested frequencies. The percentage of response was higher in both groups for the 1,000 Hz probe. The kappa test showed extremely poor agreement in the comparison of results between both probes. CONCLUSION: The occurrence of acoustic reflex was higher in newborns and its thresholds were lower with the 1,000 Hz probe both for healthy newborns and for newborns at risk.
